Tesamorelin is a synthetic analogue of GHRH (growth hormone-releasing hormone) modified with a trans-3-hexenoic acid group at the N-terminus to improve stability against dipeptidyl peptidase IV (DPP-IV) degradation. It stimulates pulsatile GH secretion from the pituitary while preserving the feedback regulation of the GH axis. Research interest in tesamorelin has focused substantially on its effects on visceral adipose tissue, where GH axis stimulation promotes preferential lipolysis of abdominal fat depots. It has been studied in the context of HIV-associated lipodystrophy and more broadly in metabolic and body composition research.
